Anterior Tricortical Iliac Bone Graft for Unstable Thoracolumbar or Lumbar Bursting Fracture with Short Segment Fixation

Abstract
Purpose: To evaluate the effect of anterior tricortical autogenous iliac bone graft with shortsegment fixation in thoracolumbar or lumbar unstable burst fracture.

Materials and Methods: Twenty patients, who had undergone anterior tricortical iliac bone graft with short segment fixation in thoracolumbar or lumbar unstable burst fracture between May 2001 and October 2010 and followed-up for more than one year, were enrolled in this study. The evaluation was performed by measuring the kyphotic angle at pre-operation, post-operation, graft union and last follow-up. We also investigated the clinical outcome at last follow-up.

Results: The kyphotic angle was preoperatively 16.3¡£, immediate postoperatively 5.6¡£, and at graft union period 7.6¡£, so about 2.0¡£of correction loss was checked. None of patients showed further progression of kyphotic deformity after graft union. At last follow-up, average VAS score was 0.8 and most patients complained bone graft site discomfort or minimal pain.

Conclusion: Anterior tricortical autogenous iliac bone graft with short segment fixation could be useful for prevention of kyphotic deformity during long term follow-up in thoracolumbar or lumbar unstable burst fracture.
